Patient and Doctor Conversation on XDR-TB treatment
Patient: Doctor, I’ve been on medication for MDR-TB, but I’m still not feeling well. Is there a way to check if the treatment is working properly?
Doctor: Yes, there’s a new test called the Genexpert XDR TB. This test is faster than traditional methods and can help us identify not only MDR-TB but also if you have any drug resistance, including pre-XDR TB. It will allow us to understand which drugs will work best for you, so we can adjust your treatment if needed
Patient: So, is XDR-TB even worse than MDR-TB?
Doctor: Yes, XDR-TB is even more dangerous. It’s a more advanced form of drug resistance. XDR-TB, an abbreviation for extensively drug-resistant tuberculosis (TB), is a form of TB that is caused by a strain of M. tuberculosis complex that is resistant to rifampicin (and may also be resistant to isoniazid), and that is also resistant to at least one fluoroquinolone (levofloxacin or moxifloxacin) and to at least one other Group A drug (bedaquiline or linezolid). This makes it much harder to treat and more challenging for doctors to find effective medications. It also carries a higher risk of side effects because we’re using stronger drugs for a longer period of time. Patient: What exactly is drug resistance, and how does this new test help with it?
Doctor: Drug resistance means that the bacteria causing TB can’t be killed by certain medications because they’ve developed resistance. Genexpert XDR TB helps identify resistance to multiple drugs, including first-line and second-line treatments. It’s crucial for adjusting your treatment plan to ensure that you get the most effective medicines, helping you recover faster and preventing complications.
Patient: What samples can my physician prescribe to test for XDR-TB?
Doctor: This is a reflex testing to be done on known positive samples of suspected MDR-TB patients. It is extremely important to understand that XDR testing is not for freshly diagnosed TB patients. It is a method of detection of extensive resistance in confirmed cases of MDR TB.
Patient: Could you please let me know what I should do if I get a 'very low' or 'trace' result on the Xpert Ultra test and Rif Resistance is present?
Doctor: It is important to note that “very low” and “trace” results on Xpert Ultra testing may come negative on Xpert MTB/ XDR testing. Contact your physician for further steps and they can get in touch with us.
.Patient: How does this test help catch pre-XDR TB?
Doctor: Pre-XDR TB is a stage where the bacteria show resistance to some, but not all, second-line drugs. Genexpert XDR TB is great at detecting this early, so we can catch it before it fully develops into XDR TB, which is harder to treat. This helps us take action sooner and prevents your condition from getting worse.
